Evaluation Form

Please use this form to give us feedback on any courses you have attended.  You do not have to limit this to KF recognised courses as we will find it useful to have comments a variety of different courses.  The information given by you on this form will be considered confidential and there is no obligation for you to provide your name.  Information gathered from these forms will only be passed to Instructors anonymously if there is seen to be a pattern to the comments and feedback left about their courses.
 
This form is not to be considered the first step in starting a formal grievance against an Instructor.  Please see the KF Code of Conduct for details about how to go about formally complaining about a KF recognised Instructor.  The KF can only consider grievances that are started within 12 months from the incident in question.
